Pennsylvania Adult Services Admissions

Individuals eligible for admission to Devereux Pennsylvania Adult Services should be at least 18 years of age. We serve people with the following diagnoses:

  • Intellectual/developmental disabilities
  • Emotional/behavioral disorders
  • Autism Spectrum Disorders
  • Brain Injury

Support for individuals with a wide range of health and physical issues is available including accessible housing. We will review each person's needs to determine if we can provide the supports and services required.

Admission Eligibility

Individuals requiring acute medical care or who can be identified as having one or more of the stated Exclusionary Criteria may exceed the level of care provided. However, applicants are evaluated and reviewed by the clinical, residential, and medical teams on a case by case basis in determining appropriateness for the particular support or service that is under consideration for admission.

Any individual whose condition or behaviors would jeopardize the safety and welfare of themselves or others may not be eligible for admission. These may include the following:

  • Individuals who are actively suicidal or psychotic and require stabilization
  • Individuals who have a history of setting multiple fires and who are identified as pathological, fire setters or have been prosecuted of arson
  • Individuals who have been convicted and charged with an offense under Megan’s Law, those who have pending or have been convicted of violent sexual offenses
  • Individuals who would jeopardize the safety and well being of others such as those who engage in extremely violent and aggressive behaviors
  • Individuals who actively use illicit drugs and abuse alcohol

For individuals receiving Pennsylvania Medicaid Waivers, they must be eligible for Social Security, Social Security for Disabled Individuals (SSDI), or Supplemental Security Insurance (SSI).  Individuals who do not have current or valid medical insurance may be denied admission.

Admissions Process

Each admission to Devereux can vary in the progression of the steps, information required, and the time needed to complete the admissions process. The Admissions Department will respond to an applicant's initial call or receipt of a referral within 24 to 48 hours, unless there is an urgent situation in which an applicant needs services immediately.

The following steps are usually involved in the Admissions process:

  • The Admissions Department can provide tours for interested persons and agencies and explain the details and steps involved in the Admission process; call us at (610) 251-2017
  • The applicant would need to confirm funding to initiate a formal referral
  • The agency or person providing the funding should submit a formal referral for consideration to the Admissions Department
  • A formal referral should include a recent Plan of Support (ISP or IEP), Psychological and/or Psychiatric Exam, Physical Exam including immunizations. Other documents will be needed prior to admission
  • A visit and/or conference call will be scheduled for members of the Devereux Team to talk with the caregiver, applicant, family and/or agency. This discussion will enable Devereux and the referring party and applicant to determine if Devereux can provide the supports and services needed. Additional meetings and visits may be scheduled throughout the Admissions process, as needed.

Person-Centered Planning

The Individual Support Plan (ISP) is completed within 30-60 days of admission. This is a collaborative process with the participation of interested family members, the individual, Adult Services’ interdisciplinary team and an individual’s representatives. Goals and outcomes are developed in the ISP and are reviewed regularly.

Discharge Planning

The process begins at the time of admission and involves the individual, the family and appropriate agency representatives. Criteria established at the Individual Support Planning Meetings are reviewed quarterly.
