School Referrals

Cleo Wallace Academy, an approved facility school, is currently accepting referrals for students in need of a day treatment program. Cleo Wallace Academy provides a therapeutic educational environment for students with social, emotional and behavioral challenges that impact their ability to be successful in a typical school environment. 

School district representatives are encouraged to email all referral documents and inquiries to: EducationIntakeCO@devereux.org 

In order for your referral to be processed efficiently, please include the following information in each referral email: 

• Student name, date of birth and grade level (required) 
• Documents for the admissions team to review - most recent IEP, ER/RR, BIP (required) 
• Any additional educational records or information deemed relevant to the referral psychiatric evaluation, attendance records, discipline records, etc. (optional and/or as requested) 

You will receive an automatic email message, confirming receipt of your referral. This email also will provide contact information for our program administrators, should you have any questions specific to the program or wish to follow up on your referral.
