Victoria Community Living ProgramThe philosophy of the Community Living Program includes teaching skills and focusing on strengths, encouraging its residents’ participation in making choices about living accommodations, school locations and work or leisure activities. Designed around person-centered planning, a process that identifies existing and natural support, the Community Living Program incorporates individual preferences and strengths, and develops strategies and resources for achieving effective outcomes. Assessment and Support PlanningEmbracing an interdisciplinary team approach, the Community Living Program emphasizes the improvement of independent functioning and access to community resources. The program staff, comprised of psychiatrists, physicians, nurses and vocational professionals, utilizes carefully designed comprehensive evaluations and initial assessments. These assessments evaluate psychiatric, physical, vocational, social and developmental, living skills, recreation, strengths and personal goals. Individual care plans are then developed that address client needs and are continuously monitored for progress and modifications. AdmissionsThe Community Living Program enrollment is limited to the number of individuals in specified target groups and Texas Department of Aging and Disability Services approved geographic areas. Private referrals are accepted for individuals with:
