Residential Referrals

Devereux Colorado’s admissions process begins with a phone call or an email to admissions staff. Each of our programs has specific placement requirements and documents needed for admissions review. If you are unsure which Devereux program might best meet your needs, our experienced admissions staff will be glad to assist you.

For residential referrals please send us the following documents via fax at (303) 438-2270, or email at referralco@devereux.org

The following documentation is needed to complete a referral review with Devereux Colorado.

  1. Current psychiatric evaluation and/or assessment
  2. Psychological testing with IQ score (if available)
  3. List of current medications, medication court order/consent
  4. Bio-psychosocial history
  5. Current physical (within the past six months) and information about any medical problems
  6. Current dental records (within the past six months)
  7. Treatment plans and progress notes from current program
  8. School information, including IEP

* Supporting documentation we will need if accepted:

  1. Birth certificate
  2. Copies of health insurance cards for medical, dental and vision
  3. Immunization records
  4. Court order of guardianship (if dependent)
  5. HIPPA release form for anyone needing a release of information (i.e., family, agency, caseworks, funders, GAL/AAL, school)
  6. Completed admissions paperwork which will be sent to you via email from an intake coordinator
