How to Initiate a Referral
CARES is a center-based day program within Devereux that provides educational services to students with autism spectrum disorders and their families. Our educational model is based on the science of Applied Behavior Analysis. |
| To initiate a referral, please refer to the guidelines below: |
| A. |
The Director of Special Education from the home school district must send a referral packet containing a letter of interest with the following attachments: |

1. |
Confirmed primary diagnosis of Autism, Aspergers or Pervasive Developmental Disorder/not otherwise specified from a medical doctor. |

2. |
Recent Evaluation Report (ER), psychological evaluation, PT/OT/SLP reports, if applicable (completed within the past 18 months). |

3. |
Copy of current I.E.P. |
| |
4. |
List and description of any secondary disabilities or medical conditions. |
| B. |
Once the referral packet is complete, CARES Administration will respond in writing regarding enrollment possibilities.
